Details of Contribution/donation

I hereby declare that details furnished by me in the donation form read with instructions as given hereunder, are true and correct to the best of my knowledge and belief. Moreover, I declare that I am an Indian resident and Indian citizen residing in India and contributing the above sum from my personal saving earned from Indian Source and thereby the aforesaid contribution is not a foreign contribution as referred here in above.

Instructions:

  • Swasthya Jagriti Sansthan do not accept any foreign contribution as defined under Foreign Contribution Regulation Act 2010 (as amended from time to time) (FCRA). Therefore, unless you are an Indian citizen and residing/living permanently in India i.e. an Indian Resident, Swasthya Jagriti Sansthan will not accept any contribution from you.
  • Swasthya Jagriti Sansthan reserves the right of not accepting your donation/contribution without assigning any reasons thereagainst.
  • On receiving communication from the Society towards acceptance of your donation application, kindly send the account payee cheque/draft at the registered office of the society.
