Terms, conditions and rules for being an associate

Applicant for being an Associate of the Swasthya Jagriti Sansthan (the Society)/Associate of the Society (hereinafter referred to as ‘the Person’ or “I” or "We”) should be above 18 years by age. The Person hereby undertakes that he/she does not have any criminal/anti‐social background.

The Person will abide by and follow the objects, rules and regulations of the Society in all the circumstances.

The person will be selected as an Associate and can be removed/rejected/disapproved/disregarded as an Associate in accordance with the rules and regulations of the Society in this respect including non‐selection as an Associate/removal etc from the Society without notice or without assigning any reason in this regard.

The Person under no circumstances can share/disseminate/copy/produce/reproduce any information/matter about or relating to the Society or its members/patron/governing body/medical advisory board/employees/committees/ Associates or any other related person or relating matter in whatsoever form, except when required/allowed to do so by the Governing Body or by any General Meeting and except so far as may be necessary in order to comply with any of the provisions of the Memorandum and Rules & Regulations of the Society.

The contact details of the applicant/Associate can be used by the Society for its purposes.

I declare that all the information furnished in this form is true and correct to the best of my knowledge and belief. I have read and understood the terms and condition and laws, rules and regulations of the Society stated/referred herein above and I hereby undertake that I will abide by said terms, conditions etc. and work in the best interest of the Society. Since this is an application for consideration of the Governing Body of the Society or the Society, I understand that I may not be considered as an Associate of the Society without assigning any reason whatsoever. I further understand that my association as an Associate of the Society can be cancelled/suspended by the Governing Body or any other relevant authority without assigning any reason thereof if my activities or conduct is deemed unfit for the Society or for any other reason, whatsoever may be. By signing this document, I agree to be part of the Society and work for it and ready to follow the terms and conditions, laws etc as stated/referred hereinabove. Disputes, if any will be subject to Kolkata, India Jurisdiction.
